Winter is approaching fast and the snow will be here before we know it! Our opening weekend for this season’s winter programs will take place on December 20 & 21.
A 2025/2026 PARTICIPANT REGISTRATION FORM AND SIGNED WAIVERS MUST BE RECEIVED PRIOR TO SUBMITTING A RESERVATION REQUEST. You can complete these forms online by clicking below:

Whether this is your first season as an ASF student or your 42nd, each student must fill out a registration form prior to reserving their lesson. Students need to fill out this form just once, no matter how many lessons they reserve.
2) WAIVERS
The ASF asks you or your parent/legal guardian to fill out waivers to participate as a student in our program. The Liability Waiver is required and this is to ensure that the student or parent/legal guardian recognizes the risks of participating in our program.
The Media Release Waiver is optional, but this would allow the ASF to use the student’s pictures and videos on its social media, website, photo galleries, marketing material, etc.
3) RESERVATION REQUEST
Once you complete the Student Participation Registration Form AND you sign the waivers above, you are now free to submit a Reservation Request!

Disclaimer: The Adaptive Sports Foundation reserves the right to cancel or postpone any lesson if, in the judgment of its management, weather conditions or a student’s physical, cognitive, or emotional condition present an unreasonable risk to the student, staff, volunteers, or others enrolled in the program.
Also, please note that the Adaptive Sports Foundation also reserves the right to adjust or change the prices of its programs and services at any time without prior notice. Current pricing at the time of registration will apply unless otherwise communicated by the Foundation.
